LPN FULL TIME DAYS AT DOCTORS HOSPITAL

BENEFIT ELIGIBLE 3 - 12 HOUR SHIFTS PER WEEK

Principal Functions:
Functions under the supervision and direction of the Nurse Manager and the Chief Clinical Officer to assist in providing and assessing direct patient care.  Participates in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the hospital and within the parameters of his or her preparation.  Cooperates with the members of the nursing and medical staffs and other hospital departments.  Relates effectively with other shifts for continuity of care. 

Position Duties:

  1. Assesses patient’s condition based upon observation and/or interview of patient, physician’s diagnosis, laboratory findings and any other pertinent information.           
  2. Uses professional judgment to prepare nursing care plans consistent with assessment of patient’s condition and physician’s treatment plans
  3. Directs non-professional personnel in implementing nursing care plans, and provides guidance and demonstrates appropriate methods, as necessary, to correct deviations from established standards.
  4. Evaluates patient’s response to nursing care and modifies or revises treatment plans, and/or contacts physicians as circumstances require.
  5. Renders professional nursing care including administering medications, and initiating and maintaining intravenous fluids and blood products according to hospital policy and procedures. Understands the pharmacology and recognizes the side effects of medications administered.
  6. Identifies problems and guides personnel to their solutions.  Communicates with other hospital departments, as necessary, in order to resolve operation problems and make the most effective use of hospital resources in delivering quality patient care.  Participates in interdisciplinary patient care conferences necessary in order to plan and evaluate effective patient care.  Performs lab draws and reports findings to appropriate personnel.
  7. Performs both complex and routine treatments as prescribed by physicians or as warranted by patient’s condition.
  8. Makes regular rounds of assigned patients independently and with physicians; observes and evaluates patient’s symptoms, progress and reactions to treatments and medications; takes corrective action as indicated.
  9. Accurately and promptly implements physician’s orders.
  10. Admits, transfers and discharges patients. Prepares patients for surgery. Gives pre-operative and post-operative care.
  11. Instructs patients and family members regarding treatments, surgical procedures, nursing plans and so forth in order to allay apprehensions; explains home care methods and techniques necessary for continuing patient care and rehabilitation.
  12. Arranges to have needed supplies and equipment on hand.
  13. Maintains the standards of accurate and complete recording and reporting.  Documents and maintains records and charts, and reports patient’s condition, medications given and treatments administered to the Registered Nurse, Nurse Manager and/or Chief Clinical Officer and the following shift; and informs physician of appropriate patient information.
  14. Demonstrates and maintains competency in nursing skills and knowledge. Demonstrates age appropriate competency skills and knowledge, displays ability to perform skills specific to patient population identified in unit specific skills list(s).
  15. Displays ability to perform skills specific to patient population identified in unit specific skills list(s).
  16. Assist in orientating and training student nurses and new personnel, and serves as a resource person for departmental personnel resolving patient care problems.
  17. Maintains competency in nursing skills and knowledge, demonstrates ability to perform skills specific to patient population identified in unit specific skill list.
  18. Floats to other units as assigned.
  19. Participates in hospital safety program.  Responsible for knowing the lifting requirements of the job and requesting assistance when appropriate
  20. Keeps Nurse Manager and/or Chief Clinical Officer informed of patients, needs, and problems on the nursing unit.
  21. Assists in reviewing and revising policies and procedures.  Observes and supports hospital policy, mission statement, and vision.  Supports and enforces infection-control policies and procedures.
  22. Contributes to the collection and evaluation of data for the nursing Quality Review Program.  Contributes to Performance Improvement activities.
  23. Floats to other departments and or facilities as directed and assigned.
  24. Maintains knowledge of current trends and developments in the field by reading appropriate books; journals and other literature and attending related seminars, conferences, etc.
  25. Maintains a professional approach with confidentiality.  Assures protection and privacy of health information as attained through written, electronic or oral disclosures.
  26. Displays concern and initiative.  Is resourceful and calm in emergencies.
  27. Is prompt and efficient with minimal absences.
  28. Cooperates and maintains good rapport with nursing staff, medical staff, other departments, and visitors.
  29. Seeks guidance and remains knowledgeable of, and complies with, all applicable federal and state laws, as well as hospital polices that apply to assigned duties.
  30. Complies with hospital expectations regarding ethical behavior and standards of conduct.
  31. Complies with federal and hospital requirements in the areas of protected health information and patient privacy.
  32. Performs other duties as assigned.

Requirements
Education:   Graduate of a technical nursing program.

Experience:   Prefer 6 months of LPN experience. Prefer experience in Acute Care Rehabilitation Nursing.

Licensure:   Currently licensed in the State of Indiana as a License Practical Nurse. CPR certification within 90 days of hire.

Skills: Excellent verbal communication skills necessary in order to instruct and comfort patients and their families. Maintain effective contacts with a variety of Hospital Personnel. Must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patient served on assigned unit/department. Must be able to lift 50 pounds and assist with 100 pounds.

Working Conditions

HOME HEALTH NURSE MANAGER

Principle Functions:
Under general direction, supervises Home Health clinical services; evaluates and determines patient needs and establishes treatment plans; coordinates quality improvement program; serves as representative to patients; receives documents, and initiates responses to patient concerns, comments, problems, and complaints.  Is responsible for patient care, management, resource management, and fiscal management. 
  
Duties and Responsibilities: 

  1. Responsible for managing, directing, and supervising the Home Health Care staff.
  2. Hires qualified personnel; disciplines, and terminated as necessary through coordination with the Chief Clinical Officer and Human Resource Director.
  3. Participates in the development and implementation of new employee orientation, including accompanying them on initial visits.
  4. Maintains current job descriptions for all positions supervised with reporting responsibilities.
  5. Maintains accurate up-to-date performance standards for employees supervised; sets yearly objectives.
  6. Maintains open communications with staff; solves problems as necessary.
  7. Provides quality ongoing educational programs for all staff supervised.
  8. Completes monthly departmental staffing schedules.  Assure staffing relevant to patient volume and workload.  Receives and handles all call offs for employees supervised.
  9. Takes referrals from multiple referral sources and coordinate necessary home care, including skilled nursing, aide services, therapies, supplies, IV therapy, social worker.
  10. Knowledge of entire patient census, new admits, discharges, resume care patients, all modalities the patient is receiving, assuring the plan of care is being carried out as ordered and revisions made as necessary.
  11. Evaluates and determines patient need; establishes and revises patient treatment plans; holds conferences with patient, families, RN’s, therapists and aides to evaluate progress and success/failure of individual treatment plans; serves as liaison between referral services, families, and patients.
  12. Develops, coordinates and implements Home Health Care quality improvement program; receives and reviews reports from all Home Health Care functions; initiates and assists in studies to monitor quality of services rendered; recommends methodology and suggests data base for studies; submits annual analysis of quality improvement reports; annually updates and/or revises quality improvement program, submits annual Home Health agency evaluations.
  13. Serves as Home Health care representative and liaison to patients, hospital, physicians, discharge planners, nursing homes, and community; applies appropriate decision making skills for the communication with field staff to provide emergency intervention; receives, investigates, and responds to patient questions, problems, and complaints; refers problems and complaints to appropriate personnel for resolution; develops, reviews, and revises policies and procedures pertaining to patient care program.
  14. Promotes a positive image of Home Health Care Services, makes home visits to assess/supervise the quality of all services provided; organize data to operate in a cost-effective manner providing quality of care.
  15. Maintains record of patient comments, concerns, and problems and identifies corrective action taken, if any.
  16. Maintains Employee incident/inquiry records, Risk Management/Quality Improvement reports.  Functions as acting chairperson of Record Review Committee with Home Health Care Administrator.
  17. Assesses patient’s condition, age, physical abilities, psychosocial status, language barriers and maintains awareness of these needs prior to and during the procedure; assesses patient’s and customers knowledge and understanding of the procedure(s) to be performed; provides education appropriate to the patient’s/customer’s age, level of comprehension, physical needs, psychological needs and social/cultural status on an individual basis.
  18. Drives to various work sites.
  19. Checks admission and discharge charts and Medicare forms prior to billing.
  20. Ensures timely submission for billing, payroll, and regulatory reports.
  21. Maintains current policies and procedures in department supervised.  
  22. Maintains a professional approach with confidentiality.  Assures protection and privacy of health information as attained through written, electronic or oral disclosures.
  23. Displays concern and initiative.  Is resourceful and calm in emergencies.
  24. Is prompt and efficient with minimal absences.
  25. Cooperates and maintains good rapport with nursing staff, medical staff, other departments, and visitors.

Requirements

Education:  Associates or Bachelors degree in Nursing from an accredited school of nursing.
Experience:  Prior supervision experience.  Prior Home Health experience.
Licensure:  Currently licensed in the State of Indiana as a Registered Nurse.  CPR certification within 60 days of hire.  Valid driver’s license with proof of vehicle insurance.
Skills:  Able to perform administrative skills.  Knowledge of theories of management.  Experience in human resource management.Skills in financial management.
